To provide the highest quality care and maintain an efficient practice, we ask that all patients review and adhere to the following policies.
Identification and Insurance
Patients are required to bring a valid photo ID and their current insurance card to every appointment. This helps ensure accurate patient identification and insurance verification.
No-Show and Late Cancellation Policy
Patients who fail to attend a scheduled appointment without notice or who cancel with insufficient notice may be subject to a no-show or late cancellation fee. Repeated missed appointments may result in dismissal from the practice.
Insurance Coverage
As a courtesy, our office verifies insurance benefits, deductibles, copays, referrals, and prior authorization requirements whenever possible. However, insurance verification is not a guarantee of coverage or payment.
Patients are responsible for understanding their insurance benefits and coverage limitations. The final determination of coverage is made by the insurance company after claims have been submitted and processed.
Patient Financial Responsibility
Patients are responsible for all charges not covered by their insurance plan, including but not limited to deductibles, copayments, coinsurance, non-covered services, and denied claims.
Cases We Do Not Accept
Our practice does not evaluate or treat patients involved in:
- Personal Injury (PI) cases
- Medico-legal cases
- Workers’ Compensation claims
Patients requiring care related to these matters should seek treatment from a provider who participates in those programs.

Verbal abuse, threatening behavior, harassment, discrimination, intimidation, rudeness, or demeaning treatment of any staff member, provider, or patient will not be tolerated. Patients who engage in such behavior may be dismissed from the practice.
